The 2023-2024 enrollment form has closed. Students mandated to enroll in annual coverage have been charged and enrolled in the plan. If you waived the annual plan, you cannot voluntarily at this time. You may qualify for special enrollment rights if you recently lost your other coverage. Please choose the Qualifying Life Event Enrollment form on the left. 

The enrollment form for the 2024-2025 academic year will be available in June.
