The 2025-2026 voluntary enrollment form is closed. The Spring 2026 voluntary enrollment form for NEW spring students is scheduled to open in November. 

If you are a current student, you may qualify for special enrollment rights. If you recently lost your other coverage, moved to the US, or recently moved from your other plan's service area and do not have comprehensive coverage near the campus, please see the qualifying event enrollment form on the left.
