The enrollment form for Fall 2025 is closed. Students who did not complete a form have been automatically enrolled and charged the student health insurance premium.

To access your online insurance card, select Insurance ID Card on the left sidebar.

The enrollment form for Spring 2025 will be available in mid-December.
