The enrollment form for the 2023-2024 academic year is closed. Students who did not complete the a form have been automatically enrolled and charged the student health insurance premium.

To access your online insurance card, please go to www.universityhealthplans.com/montclair and select Insurance ID Card on the left side of the page.

The enrollment form for Spring 2024 will be available for NEW Spring students in mid November.
