The fall 2025 waiver deadline has passed.

If you have a life changing event (i.e. involuntary loss of coverage) and need to enroll in the insurance plan after waiving, please contact University Health Plans at 1-833-251-1148 or info@univhealthplans.com. Enrollment must occur within 30 days of the qualifying life event.

The fall 2025 waiver appeal form will be available by September 26, 2025. The waiver appeal deadline will be October 22, 2025.

The spring 2026 policy period waiver form will be available in early December 2025.
