Spring 2025 Policy Period Waiver Form

Undergraduate Students: If you are registered in 9 or more credits you are automatically billed the spring insurance fee on your tuition bill and will be enrolled in the SHIP unless you submit a waiver form by the posted fall deadline. Important Note: If you are charged the insurance fee and you drop below 9 credits you will remain charged. If you do not want to be enrolled in the SHIP you must be sure to submit an online waiver form prior to the posted deadline.

All international students are required to enroll in the Student Health Insurance Plan and may not waive out of it. The insurance fee is charged to the tuition bill and international students will automatically be enrolled.

The spring 2025 policy period waiver deadline is February 23, 2025.
