The OPT enrollment deadline for the spring 2026 policy period was April 1, 2026. The form is no longer available.

Initial enrollment must occur in the semester immediately following your eligibility as a student under the Plan. If you were approved for spring 2026 OPT after the deadline, please email your explanation and date of approval to jhansen@univhealthplans.com.
