The spring 2024 policy period OPT Student enrollment deadline was April 1, 2024. The form is no longer available.

Initial enrollment must occur in the semester immediately following your eligibility as a student under the Plan. If you were approved for spring 2024 OPT after the deadline, please email your explanation and date of approval to meghan@univhealthplans.com.
