The 2024-2025 annual enrollment form is closed. Students eligible and charge for the plan have been auto enrolled and can access their insurance ID cards via the link on the left. 

If you waived the SHIP and recently lost your other coverage, choose the qualifying life event enrollment form on the left. You may qualify for special enrollment rights

The enrollment form for newly eligible spring 2025 students will open in early December.
