Summer Enrollment Form
2023-2024 Academic Year

Although students are automatically charged on their tuition statement for the SHIP, student information is not transferred to Aetna until after the deadline. Students should submit an enrollment form as soon as possible to avoid difficulties accessing their health insurance benefits. 

Students may also enroll their lawful spouse, domestic partner, and dependent children up to age 26. When dependents are enrolled, the dependent premium is paid to UHP via credit card. To enroll a dependent in summer coverage, please call UHP for assistance at 800-437-6448.

Please allow up to 7 days after submitting the enrollment form for the student (and dependent) information to be active in the insurance company's database. Students who do not submit an enrollment or waiver form by the deadline will be enrolled in the SHIP retroactively to the policy effective date and responsible for the insurance premium.
