Early Termination Form
2023-2024 Academic Year

PLEASE READ: This form should ONLY be used to terminate coverage resulting in a partial enrollment in the 2023-2024 SHIP. 

If you are a newly eligible spring student charged the spring insurance premium, choose the WAIVER FORM on the left to waive the FULL Spring Coverage Period 01/10/2024 - 08/14/2024. 

Students enrolled in the 2023-2024 Student Health Insurance Plan (SHIP) can use this form to terminate their coverage mid-academic year if they are in one of the following situations:

• not returning to Yeshiva or Cardozo School of Law for the spring semester
• enrolled in a new comprehensive health insurance plan with an effective date after 08/15/2023

Students requesting termination for the entire semester should use the waiver form on the left.
