The final deadline to confirm enrollment for the spring 2024 policy period was January 15, 2024. This form is no longer available. If you are an eligible student for spring 2024 semester charged for the insurance and did not waive, you will be automatically enrolled in the plan.

You may qualify for special enrollment rights if you previously waived the plan and recently lost your other coverage. Please contact the University Health Plans at info@univhealthplans.com for enrollment information. 

The 2024-2025 policy year waiver and enrollment confirmation form will be available after April 29, 2024.
