STUDENT HEALTH INSURANCE PLAN 
SPRING 2026 POLICY PERIOD ENROLLMENT FORM

All full-time students enrolled for a minimum of 12 credit hours will be automatically charged the spring insurance fee. Domestic students who do not intend to waive the insurance and and would like to be enrolled as well as all international students are encouraged to submit this enrollment form to confirm and expedite the processing of your enrollment. Eligibility will be confirmed with the College in January prior to processing. Student who do not take any action will not be automatically enrolled until after the deadline.

The deadline to submit the spring 2026 policy period enrollment confirmation form is January 1, 2026.
