Student Injury and Sickness Insurance Plan for

Postdoctoral Fellows, Graduate Fellows, and

Teaching and Graduate Assistants (TAs & GAs)

with Less Than Full Appointments

2014-2015 Academic Year

Dear Rutgers Postdoctoral Fellows, Graduate Fellows, and Less Than Full Appointments TA-GAs:

Rutgers University is pleased to present you with information about the Rutgers University Student Health Insurance Plan. The Student Injury and Sickness Insurance Plan is administered by University Health Plans and underwritten by UnitedHealthcare Insurance Company.

Post-Doctoral Fellows

All full-time Post-Doctoral Fellows and eligible dependents (at the fellow’s expense) will need to complete a Health Insurance Enrollment Form. Post- Doctoral Fellows in a legacy UMDNJ position at Rutgers should submit the enrollment form to the RBHS Risk & Claims Office. Post-Doctoral Fellows in a Rutgers position should submit the completed application to their supervisor or to the person who processes forms and payroll in their department.

Graduate Fellows

Graduate Fellows who have been awarded a Full Fellowship from internal or external sources are eligible. The Graduate School Dean’s Office will make final determinations as to the individual eligibility. Upon this determination, you will be automatically enrolled in the Graduate Fellows health insurance plan. No Waiver Action required; Insureds may enroll their dependents by completing the enrollment change form on the left. The premium is paid by the Insured directly to University Health Plans.

Less than Full Appointment TAs & GAs

Less than full appointment TAs/GAs are eligible under this plan as determined by University Human Resources (UHR). Once you are on payroll, call UHR at (848) 932-3990 between Spring 2015 dates between 12/8/14 – 1/23/15 to confirm employment status and ensure enrollment in the Less Than Full Appointment TA/GA health Insurance Plan. No waiver action required; you will be automatically enrolled by UHR. Insureds may enroll their eligible dependents by contacting UHR.
