The final deadline to submit the enrollment form to confirm enrollment in the Student Health Insurance Plan for the fall 2025 policy period was September 30, 2025. The enrollment form is no longer available. If you are an eligible student and did not submit a waiver or enrollment form by the deadline, you have been automatically enrolled in the Student Health Plan for the fall 2025 policy period.
