Continuation Plan Enrollment Form

Students currently enrolled in the SVA Student Health Insurance Plan for Fall 2025 who will no longer meet eligibility requirements under the policy for Spring 2026 semester are eligible to enroll in a maximum of three additional months of coverage through the SVA Student Health Insurance Plan.

The deadline to enroll in the continuation plan is February 23, 2026. There are no extensions to this deadline.
