The 2025-2026 spring waiver form is no longer available. The deadline to waive was 2/1 and students had untl 4/30 to appeal if they did not want to remain enrolled and charged in the student health insurance plan.
The summer 2025-2026 waiver form for NEW summer students is available under the Summer Waiver Form tab
The 2026-2027 annual waiver form will be available on 8/1.
Digital 1095-B tax forms will be available through the Aetna Member Portal on the Aetna Student Health website starting the last week of January, and no later than January 31st.
Students will receive an email notification at the email address on file once the form is available.
If you have not already created an account, you’ll need to select “Create Account” on the Aetna Member Portal to register before accessing your tax form.
