Use this form to request a change of address for your Blue Cross Blue Shield Student Health Insurance Plan. You should also update your mailing address with MCPHS University, if you have not already done so.

You can access a digital ID card  24/7 using the Insurance ID Card link located in the navigation column on the left hand side of the page.
