Spring Termination Request Form
Massachusetts Student Health Insurance Regulations allow students who paid for the Student Health Insurance Plan (SHIP) for the full 2025-2026 policy period to request termination for the spring coverage period if they are in one of the following situations:
- Not returning to their college or university for the spring semester
- Obtained coverage through MassHealth or a subsidized Commonwealth Connector Care plan on or before 1/1/2026
Only students in one of the two situations above can use this form. Students newly eligible for a private insurance plan do not qualify for a spring termination and will continue to utilize the student health insurance plan through 8/14/2026.
The Spring Termination Form Deadline is January 31, 2026.
Important: Students with MassHealth or a Massachusetts Health Connector Plan may not submit their request for Spring 2026 termination until they have proof of their coverage status for 2026. All current coverage under these plans only goes through 12/31/2025 and students must present coverage as of 1/1/2026 to be eligible for spring termination.
