The deadline to submit an enrollment confirmation form for the spring 2024 policy period was February 26, 2024. 

Newly eligible spring semester students who did not submit a waiver or enrollment confirmation form by the spring 2024 deadline, have been automatically enrolled in the Plan retroactive to January 1, 2024 and are responsible for the spring insurance charge on the student account. 

The 2024-2025 policy year enrollment confirmation form will be available in early summer. Please check back in June 2024 for updated information.

If you previously waived and have experienced a qualifying life event, please contact University Health Plans to discuss eligibility to enroll in the Student Health Plan.
