The 2025-2026 fall policy period waiver deadline was October 8, 2025.

Eligible students who did not waive or confirm enrollment by the deadline were automatically enrolled in the Student Health Plan and were sent an email notification. The waiver form was left available through October 21, 2025. The form has been shut down. No late waivers will be accepted at this time.
