The 2023-2024 enrollment form is closed.  The voluntary enrollment form for the 2024-2025 academic year is scheduled to open in July. 

If you have a qualifying event, such as a loss of other coverage, you may qualify for special enrollment rights. Please contact University Health Plans at 833-251-1734.
