Qualifying Event Enrollment Form
2026-2027 Academic Year
A qualifying life event is a situation that makes you eligible for special enrollment rights to enroll in coverage outside of your initial eligibility period. Eligible students may use this enrollment form to enroll if they are in one of the following situations.
Eligible Qualifying Events
LOSS OF COVERAGE
- Eligibility: Your current health insurance coverage has ended or will end soon.
- Required Documentation: A letter from your insurance carrier confirming your name and the date your coverage ends.
- Effective Date of Coverage: The first day you are no longer covered under your previous health plan.
MOVING OUTSIDE YOUR PLAN'S SERVICE AREA
- Eligibility: You are relocating outside your current health plan's service area to participate in an out-of-state clinical placement.
- Required Documentation: Documentation from your clinical coordinator verifying the location and start date of your clinical placement.
- Effective Date of Coverage: The date your out-of-state clinical placement begins.
YOUR HEALTH PLAN NO LONGER COVERS WHERE YOU LIVE
- Eligibility: Your health plan's service area has changed and no longer includes the location where you live while attending school or completing a clinical placement.
- Required Documentation: Documentation from your insurance carrier confirming the date the service area change takes effect.
- Effective Date of Coverage: The date your current health plan no longer provides coverage in the area where you live.
Students required to enroll must be logged into their Des Moines University Portal to access the enrollment form.
