Annual Waiver Form
2026-2027 Academic Year
Before requesting a waiver, confirm that your health plan meets all requirements:

Your plan must be filed and approved in the United States and fully compliant with the Patient Protection and Affordable Care Act (PPACA).
Plans that do not meet these requirements include:

Short-term medical plans
-
Limited-benefit medical plans
-
Health sharing plans
-
Travel plans
-
Plans from foreign health care systems
-
Plans marketed only to international students.
Additional requirements:

Students studying on campus: Your plan must provide non-emergency benefits in the campus area.

Students in clinical rotations: Your plan must provide national coverage. Local HMO and Medicaid plans are not accepted.
Waiver submissions will be reviewed for compliance each semester.
Students required to waive must be logged into their Des Moines University Portal to access the waiver form.
