2025-2026 Policy Year Waiver Form


Massachusetts state law requires all full-time students enrolled in a degree-granting program in the state to participate in a qualifying student health insurance program or be enrolled in a health care plan with comparable coverage. To comply with this law, all MCLA undergraduate students taking 9 or more credits and graduate students taking 7 or more credits MUST either waive or enroll in the MCLA Student Health Insurance Plan (SHIP) online. Undergraduate students taking less than 9 credits, graduate students taking less than 7 credits, students in non-degree granting programs, and students enrolled exclusively in online courses or students whose enrollment consists entirely of short-term courses are not eligible. Short-term courses are defined as courses that meet less than 15 times per semester or less than 30 times per year.

International students with a J-1 VISA are required to enroll in the Student Health Insurance Plan.

This is an annual process, and a waiver or enrollment form must be submitted each academic year to ensure that students have sufficient health insurance coverage while in attendance at the college.
