2022-2023 Policy Year Enrollment Form
August 1, 2022- July 31, 2023
The final deadline for the spring 2023 policy period enrollment confirmation form was March 10, 2023. This form is no longer available. If you are a newly eligible student for spring 2023 and did not fill out the waiver form by March 10, 2023 you will be automatically enrolled in the Student Health Plan retroactive to January 1, 2023 and responsible for the insurance charge on the student account.
If you previously submitted a waiver form and have experienced a qualifying event, such as a loss of other coverage, you may qualify for special enrollment rights. Please contact University Health Plans at 833-251-1710 for more information.
The 2023-2024 policy year enrollment confirmation form will be available later this summer. Please check the website in mid-May for updated information.

The following changes have been made to the 2022-2023 policy to keep the premium and Student Health Insurance Plan (SHIP) a viable option for students.
- Out of Pocket Maximum will increase to $8,700 for combined medical and prescription expenses.
- The prescription drug copayments will change to a 5-Tier Plan:
- Tier 1: $10 copay (generic)
- Tier 2: $20 copay (preferred brand)
- Tier 3: $30 copay (non-preferred brand)
- Tier 4: 30% coinsurance (preferred specialty)
- Tier 5: 30% coinsurance (non-preferred specialty)
The plan design changes can have an impact on your out-of-pocket costs if you need a preferred or non-preferred specialty prescription that will require you to pay 30% of the medication's cost up to the out-of-pocket maximum.Before enrolling, we encourage students impacted by the change to compare other insurance options to the SHIP.
To look-up your prescription medication in the 5-Tier Plan, choose Prescription Info → Medication Look Up in the navigation menu on the left.
The Massachusetts Health Connector offers information about their online marketplace where you can compare and purchase insurance plans. You may be eligible for a Subsidized Health Plan (a low or no cost insurance program for qualifying uninsured adults).
